Postmortem: Orchestrator and Vision Engine Failures

On October 16th, 2026, our systems experienced a series of failures across the orchestrator and vision engines. This resulted in multiple items being routed to the dead_letter table with ‘all_engines_failed’ errors. Our investigation points to recent changes in content correlation and fleet-wide sanitization as the primary contributors. We have implemented a fix to address the underlying issues and prevent recurrence.
